How can I confirm that I am eligible for Home Health Care?

Patients are eligible for home health services if they are homebound. A patient is considered homebound if:

• The patient has an illness or injury that restricts the ability to leave the home environment except with the assistance of another individual or the aide of a supportive device (such as crutches, a cane, a wheelchair or a walker) or if the patient has a condition that is medically contraindicated.

• The condition should be such that there exists a normal inability to leave home and leaving home requires a considerable and taxing effort.

Acceptable reasons for leaving home include the need to receive health care treatment, attend an adult day care program, or attend religious services. Other absences shall not disqualify an individual if the absence is infrequent or of a relatively short duration.
