Coordination of Care


 

At Noble Home Health, our goal is to provide comprehensive care that is focused on the overall health of the patient. To accomplish this, we focus on coordination of care with the patient, physicians and caregivers.

Upon admission, our clinician will assess the patient to determine the best plan of care. We will then work with the patient and their physician to set goals to help them reach their optimal level of health.

Fall Prevention


 

Falls are the leading cause of fatal injury among older adults(65 and older). One in four Americans age 65+ falls each year. Every 11 seconds, an older adult is treated in the emergency room for a fall. Every 19 minutes, an older adult dies from a fall.

Our program includes assessment, training and exercises to help prevent falls. Our clinical team will assess the patient’s medication and develop specific training programs to improve balance, gait and safe transfers. The clinician will also assess the patient’s home and supportive device(s) then recommend improvements as needed.

Medication Management


 

Medication management can be a key derailer in achieving optimal health. Our clinicians work with the patient and their physician to ensure they are correctly taking their medication as prescribed.

Other Services


 

Diabetes Management. Our clinicians work with our patients to educate them on how to manage diabetes and live a better quality of life.

Alzheimer’s and Dementia Care. One in three seniors dies of Alzheimer’s or another form of dementia. This condition can have a taxing effect on the patient and the loved ones who care for them. Our clinical staff will work with the family on education and customize a treatment plan to help the patient and caregiver have a better quality of life.

Insurance and Service Area


 

Insurance. We work with Medicare, private insurance providers and accept self-pay patients. Part of the admission process includes verification of insurance. We can help answer your financial questions and help you navigate the insurance process.

Service Area. We service the Greater Houston area with a focus on the Harris, Katy and Fort Bend areas. However, we are not limited to these areas and evaluate each patient on a case-by-case basis. We strive to provide care to all patients in need.
